Swol Painkiller

SWOL Painkiller

The Painkiller is a creamy tiki classic, and it is traditionally made with rum. Our SWOL Painkiller swaps in SWOL Añejo Tequila for a tequila Painkiller with a little more oak and agave behind the coconut. Pineapple and orange juice bring the tropical sweetness, cream of coconut makes it rich and smooth, and a dusting of fresh nutmeg on top ties it together. It is one of the easiest tequila cocktails to make: everything goes in the shaker, and the harder you shake, the silkier it gets. Below is the recipe, how the tequila version compares to the rum original, and the coconut ingredient that makes or breaks it.

INGREDIENTS
  • 2 oz SWOL Añejo Tequila, the hero; oak and agave stand in for the classic rum.
  • 1 oz pineapple juice, for tropical body.
  • 1 oz orange juice, fresh for brightness.
  • 1 oz cream of coconut, the sweet, thick coconut base (not coconut milk or water).
  • Fresh nutmeg, grated over the top; the signature Painkiller aroma.
  • Garnish: pineapple wedge or leaf.
HOW TO PREPARE
  1. Add the tequila, pineapple juice, orange juice and cream of coconut to a shaker with ice.
  2. Shake hard for a good 15 seconds so the cream of coconut fully blends.
  3. Strain into a hurricane glass over fresh crushed ice.
  4. Grate fresh nutmeg generously over the top.
  5. Garnish with a pineapple wedge or leaf.

Tequila Painkiller vs the classic

The original Painkiller is a Pusser's rum drink. Swapping in SWOL Añejo keeps the pineapple, orange and cream-of-coconut formula but adds oak and agave, making a slightly drier, more complex version. Everything else about the build stays true to the classic.

Cream of coconut, explained

Cream of coconut is sweetened and thick (think Coco Lopez), and it is what gives a Painkiller its rich texture. Do not swap in coconut milk or coconut water; they are thinner and unsweetened and will make a watery drink. Stir the can before measuring, since it separates.

Batch it for a crowd

Multiply each ingredient by the number of guests and whisk or blend the cream of coconut in well so it does not clump. Keep chilled and shake or stir each pour over ice, finishing every glass with fresh nutmeg.

Frequently Asked Questions

Can you make a Painkiller with tequila?

Yes. A tequila Painkiller swaps the classic rum for añejo tequila, keeping pineapple, orange, cream of coconut and nutmeg.

What is cream of coconut?

A sweetened, thick coconut product that gives the Painkiller its creamy body; it is not the same as coconut milk or water.

What tequila is best?

An añejo such as SWOL Añejo, whose oak balances the sweet coconut.

Why grate fresh nutmeg on top?

Fresh nutmeg is the Painkiller's signature aroma and lifts the creamy coconut.

Can I use coconut milk instead?

Not recommended; it is thinner and unsweetened and will make the drink watery. Use cream of coconut.

How is it different from a piña colada?

A Painkiller adds orange juice and skips blending, so it is looser and more citrusy than a frozen piña colada.

Can I batch it for a party?

Yes; scale each ingredient and blend the cream of coconut in well, then finish each glass with nutmeg.

Swol Painkiller TIPS

Shake Hard & Spice Right

Shake vigorously to fully blend the cream of coconut — it creates that creamy, smooth texture the Painkiller is known for. Always use freshly grated nutmeg on top; the aroma hits first and adds a warm, spicy contrast to the tropical fruit and añejo richness. Want to go extra luxe? Float a splash of overproof rum or mezcal on top for an island-style kicker.
